Notice of Privacy Practices

Blue Lotus Longevity Clinic is committed to protecting the privacy of your protected health information (PHI). This Notice of Privacy Practices (the "Notice") explains our legal duties and privacy practices concerning your medical information, as required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and California state law. We are required to abide by the terms of this Notice.

For example, a physician may share your medical history with a physical therapist to coordinate a treatment plan.

This may include, but is not limited to, quality assessment and improvement activities, training, licensing, and other administrative and legal services. We may also use your information to contact you with appointment reminders or to inform you about health-related products and services that may be of interest to you.

• Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures of your PHI we have made. The list will not include disclosures for treatment, payment, or healthcare operations.
• Right to Request Restrictions: You have the right to request a restriction on how we use or disclose your PHI for treatment, payment, or healthcare operations. We are not required to agree to all restrictions, but if we do, we will be bound by that agreement.
• Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location. For example, you can ask that we only contact you at work or by mail.
• Right to a Paper Copy of This Notice: You have the right to a paper copy of this Notice, even if you have agreed to receive an electronic copy. You may request a copy at any time.
• Right to Be Notified of a Breach: We are required to notify you in the event of a breach of your unsecured PHI.

6. Our Responsibilities

• We are required by law to maintain the privacy and security of your PHI.
• We will provide you with a copy of this Notice.
• We will abide by the terms of this Notice.
• We reserve the right to change our privacy practices and the terms of this Notice at any time. Any changes will apply to all PHI we maintain. We will post a revised Notice on our website and in our office.

7. Complaints

If you believe your privacy rights have been violated, you may file a complaint with Blue Lotus Longevity Clinic or with the Secretary of the U.S. Department of Health and Human Services. To file a complaint with us, please contact Dr. Omron directly.

We will not retaliate against you for filing a complaint.
